Medical records request letter you can mail online
Need to request medical records from a doctor, hospital, clinic, or release-of-information department without printing anything yourself? PostalForm now has a dedicated Medical Records Request Packets workflow that builds the request letter, organizes optional authorization files, shows the final PDF packet, and mails it to the provider address you enter.

How it works
Choose the request type
Use the workflow for your own records, an authorized representative request, records sent to a new provider, or billing and insurance records.
Enter patient and requester details
Add the patient name, date of birth, requester relationship, reply contact information, and return address.
Describe the records
Select chart records, visit notes, labs, imaging, billing records, immunizations, or other records, then add the date range and release instructions.
Attach support if needed
Upload a provider authorization form, ID copy, relationship proof, prior request, or other supporting files.
Preview before checkout
PostalForm generates a cover page, request letter, summary page, exhibit index, and attachments so you can review the packet before paying.
Mail the packet
Enter the provider mailing address, choose mailing options, and PostalForm prints and mails the request.
What to include in your request
- Patient full name and date of birth.
- Requester name, relationship to the patient, reply email, phone, return address, and signature.
- Provider, hospital, clinic, medical records, or health information management mailing address.
- Specific record types and date ranges.
- Preferred delivery method for the records, such as portal, encrypted email, fax, mailed copies, or the provider's standard method.
- Authorization form, ID copy, guardianship proof, power of attorney, or provider-specific form when the provider requires it.
Common mistakes
- Mailing to a general clinic address instead of the medical records, health information management, or release-of-information department.
- Asking for "everything" when the provider needs a date range or specific record type.
- Requesting release to another person or organization without attaching the authorization details the provider requires.
- Forgetting a signature, requester contact details, or patient date of birth.
- Paying before reviewing the generated packet and provider address.
This page is informational only and does not provide medical or legal advice. PostalForm prepares and mails the packet; the provider controls how it verifies identity, processes the request, and releases the records.
When mailing makes sense
Many providers offer a portal, and that should usually be the first option when it works. HHS says people generally have the right to see and get copies of health information, and HealthIT.gov notes that request methods depend on the provider process, including portal, form, email, mail, or fax. A mailed request is useful when:
- The provider asks for a signed written request.
- You need to attach an authorization form, ID copy, or representative proof.
- The records department publishes a mailing address for requests.
- You want a packet you can review and keep for your records.
- You want optional Certified Mail proof that the request reached the provider.

FAQs
- Do I need to sign the request?

- Can I request records be sent to someone else?
- Yes, if the provider accepts the authorization. Clearly name who should receive the records and attach any provider-required release form.
- Can I attach an ID or authorization form?
- Yes. The workflow accepts optional PDF, JPG, JPEG, and PNG attachments for authorization forms, ID copies, relationship proof, prior requests, and supporting records.
