Where to mail it (don't guess)
- Use the address on the letter that requested the documents (best).
- If you only have a general insurance address, look for a claims, subrogation, third-party liability, or COB department address.
- If unsure, call the number on the insurance card or the letter and ask: "What is the mailing address for accident-related claim attachments?"
Some insurers call these accident-related claim packets "third-party liability," "subrogation," or "coordination of benefits (COB)" paperwork.
Recommended packet order
- Cover letter (lists everything enclosed).
- Claim form (completed and signed if required).
- Itemized medical bills (grouped by provider or date).
- EOBs in the same order as the bills.
- Any accident or third-party documents the insurer requested.

- Use the mailing address on the letter that requested the documents.
- Make scans legible and full-page (no cut-off corners).
- Include your Member ID and claim number on the cover letter.
