SSA-44 Customer-Assembly Preparation Kit
This workflow helps you prepare an SSA-44 request to reduce Medicare IRMAA after a qualifying life-changing event. PostalForm prepares the official form and delivers the unsigned preparation kit only to your verified address. PostalForm does not file, serve, or submit it. Hand-sign the form, add the branch-specific evidence SSA requires—including originals or certified copies where required—and personally submit the complete request to the local Social Security office.

Important details+
Answer the guided questions and PostalForm prepares the official form, leaves signature areas blank, adds a customer instruction cover, and sends the packet only to your verified address. Nothing is filed or submitted for you. Hand-sign the form, add the branch-specific evidence SSA requires—including originals or certified copies where required—and personally submit the complete request to the local Social Security office.
What you need+
- The life-changing event and the month/year it happened
- Your name and Social Security number
- Adjusted gross income and tax-exempt interest for the more recent tax year (estimates are allowed)
- Your tax filing status for that year
- Signature, phone number, and current mailing address
- Hand-sign the printed form after the preparation kit is delivered
- Signed copy of your federal tax return (only if you already filed for that year) (optional) — If you already filed a federal return for the tax year in your request, attach a signed copy (or an IRS transcript). If you used an estimate because you haven’t filed yet, skip this — SSA will ask for the signed return after you file it.
- Proof of the life-changing event (optional here — read the note) (optional) — For a work stoppage or reduction (like retirement), nothing is required — your signed statement on the form is accepted, though a copy of an employer letter or final pay stub helps. For marriage, divorce, a death, property loss, pension loss, or an employer settlement, SSA must see ORIGINAL documents or certified copies and will mail them back; printed copies attached here usually are not enough, so plan to show or send those documents to your local office separately.

How it works+
- Answer the guided questions and review the prepared form information.
- PostalForm prints the official form with signature areas left blank and sends the customer-assembly kit only to your verified address.
- After delivery, follow the cover instructions, hand-sign where required, and add every original, certified record, payment, witness, certification, companion form, or other physical item.
- Verify the current deadline and destination, then personally complete final submission through the official channel.
Where it gets mailed+
PostalForm mails the unsigned preparation kit to you. You complete the signatures, physical enclosures, and final submission yourself.
Packet order+
- Customer assembly instruction cover (remove and keep before final submission)
- Prepared unsigned form (signature and reserved third-party fields blank)
- Printed reference attachments
Common mistakes+
- Filing when your income is already below the IRMAA threshold — the form itself says not to file if no surcharge applies to you.
- Entering a four-digit year in the tax-year boxes — the form pre-prints “20” and only has room for the last two digits.
- Mailing the form to SSA headquarters in Baltimore instead of your local Social Security office.
- Using SSA-44 to report an amended tax return or an IRS error — SSA handles those corrections without this form; call 1-800-772-1213 instead.
- Forgetting that marriage, divorce, and death events need original or certified documents, which SSA must see and will return.
FAQs+
What is IRMAA and why is my Medicare premium higher?
IRMAA (income-related monthly adjustment amount) is a surcharge added to Medicare Part B and prescription drug premiums for people above certain income levels. SSA sets it using IRS data from about two years ago — so your first years of retirement can be charged as if you were still working.
Which events count as life-changing events?
Marriage, divorce or annulment, death of your spouse, work stoppage (such as retirement), work reduction, loss of income-producing property, loss of pension income, and an employer settlement payment. A one-time spike like a home sale or IRA withdrawal does not qualify.
Where do I send Form SSA-44?
To your local Social Security office — there is no national mailing address. Find your office’s address at ssa.gov/locator or call 1-800-772-1213. You can also fax it to your local office, bring it in person, or skip the form entirely by phoning SSA for an appointment.
Can I submit SSA-44 online instead of mailing it?
Yes. SSA lets you complete and upload the form and your evidence through a my Social Security account (see ssa.gov/medicare/lower-irmaa). Mailing the signed paper form works just as well if you prefer paper — that is what this service prepares.
What evidence do I need to include?
Two things: proof of your lower income (a signed copy of that year’s tax return if you have filed — an estimate is fine if you have not), and proof of the event. For a work stoppage or reduction such as retirement, your signed statement on the form itself is accepted. For marriage, divorce, a death, property loss, pension loss, or an employer settlement, SSA must see original documents or certified copies, which they return to you.
Should I mail original marriage or death certificates?
Be careful: SSA requires originals or certified copies for those events and will return them, but many people prefer to show originals at a local office. The preparation kit cannot supply those physical originals; add or present them yourself during final submission.
What happens after SSA gets my form?
SSA reviews your request, verifies the income you reported with the IRS, and sends you a written decision. If SSA agrees, your premium is corrected and any extra IRMAA you already paid for affected months that year is refunded or credited. If you disagree with the decision, you can appeal by requesting a reconsideration.
I filed my taxes as married filing separately but lived apart from my spouse all year. Should I use this form?
No. The form instructions say to call SSA at 1-800-772-1213 (TTY 1-800-325-0778) to report that situation instead of filing SSA-44.
