Do I mail PS Form 1583 to USPS?
No. You execute it with your mailbox provider (CMRA). The provider witnesses or verifies your signature, keeps a copy at their location, and uploads the original to USPS's CMRA Customer Registration Database. USPS does not accept customer 1583s by mail.
Does Form 1583 have to be notarized?
Not necessarily. Under the current rules you may sign in the physical presence of your CMRA, sign in their virtual presence over real-time audio/video, or acknowledge your signature before a notary commissioned in a U.S. state or territory. Many virtual mailbox providers offer built-in online verification so no separate notary visit is needed.
What identification do I need?
Two IDs: one government-issued photo ID (driver's or non-driver's ID, passport, uniformed service ID, certificate of naturalization, permanent resident card, Matricula Consular, U.S. Access Card, U.S. university ID, or NEXUS) and one document that proves your home address (lease, mortgage or deed of trust, insurance policy, vehicle registration, or voter card).
